in
Sudbury
County of Middlesex
State of Massachusetts

At Readville, Mass. on the seventeenth day of September, 1862, I entered the service, as private, in Company F. 45th Mass. Vol. And at the close of the war my rank was still the same.

On the seventh of June, 1863, I was discharged as my term of service had expired and was never again enlisted.

The first battle I took part in was at Kinston, North Carolina. I was also engaged at White Hall, Goldsboro, Young’s Cross Roads and a number of small engagements.

I never was wounded, confined in hospital or taken prisoner.

Among my most intimate comrades were A. Puffer and J. H. Eaton.

I Certify that the Sketch of my War service as above written is true as
I verily believe
